The CurerTech Patient Portal is a healthcare application that allows patients to: • View and manage appointments • Access lab results and medical records • Communicate with healthcare providers • View and manage insurance information • Upload and store medical documents • Receive health-related notifications

IMPORTANT: This App is NOT a medical device and is NOT intended to: • Diagnose, treat, cure, or prevent any disease • Replace professional medical advice • Provide emergency medical services Always consult your healthcare provider for medical decisions. In case of a medical emergency, call 911 or your local emergency services immediately. The information provided through this App is for informational purposes only and should not be considered medical advice.

To use this App, you must: • Be at least 18 years of age • Be a registered patient with a CurerTech-affiliated healthcare provider • Have valid login credentials provided by your healthcare provider • Agree to these Terms and our Privacy Policy

Account Security: • You are responsible for maintaining the confidentiality of your login credentials • You must notify us immediately of any unauthorized use of your account • We recommend enabling biometric authentication when available Account Accuracy: • You agree to provide accurate and complete information • You are responsible for updating your information as needed

You agree NOT to: • Share your login credentials with others • Attempt to gain unauthorized access to the App or its systems • Use the App for any illegal or unauthorized purpose • Interfere with or disrupt the App's functionality • Upload malicious software or harmful content • Impersonate another person or entity

Your Health Data: • Health information displayed in the App comes from your healthcare providers • We do not independently verify the accuracy of medical information • Contact your healthcare provider directly for questions about your health data Data Accuracy: • While we strive for accuracy, we cannot guarantee that all information is error-free • Report any discrepancies to your healthcare provider

• The App and its content are owned by CurerTech • You may not copy, modify, or distribute the App without permission • CurerTech and associated logos are trademarks of CurerTech

The App may integrate with third-party services: • Healthcare provider systems • Laboratory information systems • Cloud storage services (AWS) We are not responsible for the availability or accuracy of third-party services.

To the maximum extent permitted by law: • The App is provided "AS IS" without warranties • We are not liable for any indirect, incidental, or consequential damages • Our total liability shall not exceed the amount paid by you for the App (if any)

You agree to indemnify and hold harmless CurerTech from any claims arising from: • Your use of the App • Your violation of these Terms • Your violation of any third-party rights

By You: • You may stop using the App at any time • You may request account deletion through the App or by contacting support By Us: • We may suspend or terminate your access for violation of these Terms • We may discontinue the App with reasonable notice

• We may update these Terms from time to time • Continued use of the App after changes constitutes acceptance • Material changes will be notified through the App

These Terms are governed by the laws of the jurisdiction where CurerTech operates, without regard to conflict of law principles.

Any disputes arising from these Terms shall be resolved through: 1. Good faith negotiation 2. Mediation 3. Binding arbitration (if required)

If any provision of these Terms is found unenforceable, the remaining provisions will continue in effect.
